We here at Louisiana Hemophilia Foundation strive to see every individual with a bleeding disorder have a Medical ID along with membership services in the event of an emergency. LHF will provide this service to you at no cost. Once enrolled your membership services will last a life time. However if yur medical condition or medications have changed it is the patient's responsibility to update that information in their account. Simply click the PDF below, fill it out, and email it back to us at Outreach@Lahemo.org. The ID of your choice will be shipped directly to the you and we cover the cost. Membership services are available once application is processed. We also accept applications on your behalf from your HTC or Hematologist sent to us using the above email address.
